SPORTS REGISTRATION - FINALFORMS

2024 SPRING SPORTS REGISTRATION IS OPEN

SHS Spring Sports:  Baseball, Softball, Golf, Boys/Girls Lacrosse, Boys/Girls Tennis, Boys/Girls Track and Field, Boys Volleyball

SIS Spring Sports:  Baseball, Softball, Boys Outdoor Track, Girls Outdoor Track

For RETURNING ATHLETES:  Log in to your FinalForms account, and choose the appropriate Fall Sport.   All forms must be reviewed and signed each year.

For NEW REGISTRATIONS:  Click on: "Sign Up to Play Sports"  to register on FinalForms.  Follow the prompts to create your parent account, create your student's account, and sign your forms. 

***students can only be registered  for ONE sport per season***                                    **PLEASE ONLY REGISTER STUDENTS CURRENTLY IN GRADES 7-12 !! **

Physicals CANNOT be uploaded to Final Forms.     PHYSICALS can be securely e-faxed to 203-567-8534; emailed to bhansen@sheltonpublicschools.org; dropped off directly at the Shelton High School athletic office; or for SIS students, given to the SIS nurses.

Shelton Athletics uses FINALFORMS, which allows you to register for school sports and complete and sign athletic participation forms for your students.  FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice.  FinalForms also pre-populates information wherever possible, for each of your students, saving you time.  You may review your data at any time to verify it is current.  You will be required to sign your forms once per year and after any update.  Thank you for your assistance in streamlining our paperwork process at Shelton High School and Shelton Intermediate School.

GoFan Digital Tickets for ALL Shelton home games

Gate tickets are required for all HOME athletic events in Finn Stadium and Murray Gymnasium.  Avoid the lines and please purchase your GoFan tickets using this link GoFan Shelton Tickets or by going to GoFan.co and searching for Shelton High School.   20-game and 10-game passes are also available on GoFan.

General Admission (Adult & Student) tickets are $6.00 and Senior Citizen tickets are $3.00.   We will continue to accept Cash at our sporting events at this time; however, it will be the same price for Cash tickets or GoFan Tickets. 

Only one ticket per person is required for entry to a JV and/or Varsity game.  

Shelton High School students are able to buy a student pass in person at the athletic office for $30.00 (cash/check only) that will allow them into all regular season home games for all sports for the remainder of the school year.  Please see Mrs. Hansen in the Athletic Office to purchase a student pass.

  • SPORTS PHYSICALS

    All athletes must have a current valid physical form on file prior to trying out or practicing with any sports team.  Physicals are valid for 13-months from the date of exam, and must be valid to participate in any tryout, practice, and/or game.  Athletes must renew their physical form each year.

    Sports Physicals are available at many walk-in clinics in the area.  There is a nominal charge for sports physicals at these locations.   Download the sports physical below, complete the history page, and bring with you.

    The SHS Athletic Department is excited to bring LIVE VIDEO feeds of all Home games held in Finn Stadium or Murray Gymnasium for every game at every level.  Parents, friends, and fans of SHS Athletes can subscribe to the NFHS Network to watch all HOME games streamed LIVE to their computer, tablet, or mobile phone through the NFHS Network.  Details can be found at the NFHS Network , search for "Shelton High School."

Physical Examination Policy

All athletes must have a current valid physical on file with the nurses office prior to trying out or practicing with any sports team.    Per CIAC policy, physicals are valid for 13 months from the date of exam, and must be valid to participate in any tryouts, practices, and/or games.   Athletes must renew their physical every year.   THERE ARE NO EXCEPTIONS TO THIS POLICY.
